The Independent Police Investigative Directorate (IPID) lead investigator expressed dissatisfaction over the testimony of a key witness, known as Witness D, in the case against suspended Ekurhuleni police chief Julius Mkhwanazi. Witness D, who had previously survived assassination attempts, testified about the torture and death of Emmanuel Mbhense by EMPD and SAPS officials, and alleged that Mkhwanazi was involved in disposing of the body. The lead investigator had taken Witness D's statement in 2022 and kept it secret to protect him. Witness D was later killed in December 2025.
